OBSTRUCTIVE JAUNDICE

Obstructive Jaundice

Causes, Symptoms, Diagnosis, Complications & Advanced Treatment Options

Advanced Hepatobiliary & Laparoscopic Surgical Care in Thane – By Dr. Ankur Bhanushali

What Is Obstructive Jaundice?

Obstructive jaundice is a condition in which the normal flow of bile from the liver to the intestine is blocked. This blockage causes bile pigments (especially bilirubin) to accumulate in the blood, leading to yellow discoloration of the skin and eyes.

Unlike medical jaundice caused by liver cell damage, obstructive jaundice occurs due to a physical blockage in the bile ducts. It is a serious condition that often requires urgent evaluation and intervention.

How Does Obstructive Jaundice Occur?

Normally:

1️⃣ Liver produces bile
2️⃣ Bile flows through bile ducts
3️⃣ Stored in gallbladder
4️⃣ Released into intestine for digestion

When the bile duct is blocked:

✔ Bile cannot drain
✔ Pressure builds up in liver
✔ Bilirubin enters bloodstream
✔ Jaundice develops

If untreated, it may lead to infection, liver damage, or life-threatening complications.

Common Causes of Obstructive Jaundice

1️⃣ Gallstone Disease (Most Common Cause)

✔ Stone in common bile duct (CBD stone)
✔ Migrated gallbladder stones

2️⃣ Tumors

✔ Pancreatic head cancer
✔ Cholangiocarcinoma (bile duct cancer)
✔ Ampullary carcinoma
✔ Gallbladder cancer

3️⃣ Benign Conditions

✔ Bile duct strictures
✔ Post-surgical injury
✔ Chronic pancreatitis

4️⃣ Infections & Inflammation

✔ Acute cholangitis
✔ Parasites (rare)

Symptoms of Obstructive Jaundice

✔ Yellow discoloration of eyes and skin
✔ Dark urine
✔ Pale or clay-colored stools
✔ Itching (pruritus)
✔ Right upper abdominal pain
✔ Fever (if infection present)
✔ Loss of appetite
✔ Weight loss

Dangerous Warning Signs 🚨

Immediate medical care is required if there is:

🚨 High fever with chills (suggests cholangitis)
🚨 Severe abdominal pain
🚨 Persistent vomiting
🚨 Confusion or drowsiness
🚨 Sudden worsening of jaundice

Obstructive jaundice with infection is a medical emergency.

How Is Obstructive Jaundice Diagnosed?

Accurate diagnosis requires clinical and radiological evaluation.

Blood Tests

✔ Elevated bilirubin
✔ Raised alkaline phosphatase
✔ Abnormal liver function tests

Imaging Studies

✔ Ultrasound abdomen (first-line test)
✔ CT scan abdomen
✔ MRCP (Magnetic Resonance Cholangiopancreatography)
✔ Endoscopic ultrasound (EUS)

Endoscopic Evaluation

✔ ERCP (for both diagnosis and treatment)

Treatment of Obstructive Jaundice

Treatment depends on the underlying cause.

1️⃣ ERCP (Endoscopic Retrograde Cholangiopancreatography)

Most commonly performed procedure.

✔ Stone removal
✔ Stent placement
✔ Bile duct drainage
✔ Relieves obstruction

Often done as a daycare or short-stay procedure.

2️⃣ Laparoscopic Surgery

✔ Laparoscopic cholecystectomy
✔ CBD exploration
✔ Removal of obstructing stones

3️⃣ Radiological Drainage

✔ Percutaneous transhepatic biliary drainage (PTBD)
✔ Used when ERCP is not possible

4️⃣ Cancer-Related Treatment

If caused by tumor:

✔ Surgical resection (Whipple procedure in pancreatic cancer)
✔ Biliary bypass surgery
✔ Chemotherapy
✔ Palliative stenting

Management of Itching & Symptoms

✔ Bile acid-binding medications
✔ Skin care measures
✔ Nutritional support

Recovery & Follow-Up

✔ Monitoring liver function
✔ Repeat imaging if needed
✔ Removal or replacement of stents
✔ Follow-up for underlying condition

Early intervention usually leads to excellent recovery in benign causes.

Possible Complications If Untreated

❌ Acute cholangitis
❌ Liver failure
❌ Sepsis
❌ Kidney failure
❌ Death

Timely treatment significantly reduces complications.

Prevention of Obstructive Jaundice

✔ Early treatment of gallstones
✔ Regular follow-up for liver disease
✔ Avoid self-medication
✔ Early evaluation of persistent jaundice

When Should You See a Specialist?

✔ Yellowing of eyes or skin
✔ Dark urine for more than 2–3 days
✔ Persistent itching
✔ Abdominal pain with jaundice
✔ Unexplained weight loss

Early diagnosis improves outcomes and prevents serious complications.
