MILD PERITONEAL THICKENING & ENHANCEMENT IN THE PELVIS

Mild Peritoneal Thickening & Enhancement in the Pelvis – What Does It Mean?

When a scan (CT/MRI/USG) shows β€œmild peritoneal thickening and enhancement in the pelvis,” it means there is slight inflammation of the peritoneum β€” the thin protective layer lining the inside of the abdomen and covering abdominal organs.

πŸ‘‰ It is NOT a disease, but a sign that something is irritating the pelvic peritoneum.

πŸ”¬ What Is the Peritoneum?

A thin membrane that:

  • Covers abdominal & pelvic organs
  • Protects against infection
  • Helps organs move smoothly

When irritated β†’ it becomes thicker and shows up brighter (β€œenhanced”) on scans.

Possible Causes of Mild Pelvic Peritoneal Thickening

1. Pelvic Inflammatory Disease (PID)

Common in women; infection of uterus, ovaries, fallopian tubes.

2. Recent Pelvic or Abdominal Infection

Even mild infections can leave inflammatory changes on scans.

3. Early or Resolving Appendicitis

Appendix-related inflammation may extend to pelvic peritoneum.

4. Abdominal Tuberculosis (TB)

Very common in India; may show mild thickening + minimal fluid.

5. Endometriosis

Endometrial tissue outside the uterus causes chronic irritation.

6. Ovarian Cyst Rupture or Leakage

Fluid irritates the peritoneum temporarily.

7. Post-surgical Changes

After C-section, D&C, laparoscopy, hernia repair, etc.

8. Mild or Early Peritonitis

If symptoms are present.

9. Non-specific / Incidental Finding

Sometimes seen without any serious cause.

Symptoms You May Notice

  • Lower abdominal or pelvic pain
  • Bloating or discomfort
  • Fever (if infection)
  • Painful periods (if endometriosis)
  • Irregular bowel changes

Some people have no symptoms at all.

Tests Usually Recommended

Your doctor may advise:

  • CBC
  • CRP / ESR
  • Urine test
  • Vaginal swab (if PID suspected)
  • TB-PCR / Mantoux / GeneXpert (if TB suspected)
  • Repeat ultrasound
  • CT/MRI if symptoms worsen

Treatment Depends on the Cause

There is no single treatment. Management is based on the underlying reason:

  • Antibiotics β†’ for PID or infection
  • Anti-inflammatory medicines β†’ for mild irritation
  • Anti-TB therapy β†’ if TB confirmed
  • Hormonal therapy β†’ if endometriosis
  • Observation only β†’ if asymptomatic & mild

A doctor will correlate with symptoms before deciding.

⚠️ When to Seek Immediate Medical Help

  • Severe abdominal pain
  • High fever
  • Vomiting
  • Increasing abdominal swelling
  • Sudden worsening pelvic pain

These may indicate progressing peritonitis.
