Advanced Emergency & Laparoscopic Surgical Care in Thane – By Dr. Ankur Bhanushali
What Is an Appendicular Abscess?
An appendicular abscess is a localized collection of pus around the appendix, usually occurring as a complication of untreated or delayed acute appendicitis.
When appendicitis is not treated in time, the inflamed appendix may perforate. In some cases, instead of widespread infection throughout the abdomen, the body attempts to contain the infection by forming a localized abscess (a pocket of pus).
Although it may appear “contained,” an appendicular abscess is still a serious condition and requires expert medical and surgical management.
How Does an Appendicular Abscess Develop?
The typical sequence:
1️⃣ Acute appendicitis begins
2️⃣ Inflammation worsens
3️⃣ Appendix perforates (ruptures)
4️⃣ Infection spreads locally
5️⃣ Body forms an abscess to contain infection
Delayed diagnosis, self-medication, or ignoring symptoms increases the risk.
Common Causes
✔ Untreated acute appendicitis
✔ Delayed hospital consultation
✔ Incomplete antibiotic treatment
✔ Weak immune system
✔ Diabetes mellitus
Symptoms of Appendicular Abscess
Symptoms may initially resemble appendicitis but become more severe or prolonged.
✔ Persistent right lower abdominal pain
✔ High-grade fever
✔ Loss of appetite
✔ Nausea and vomiting
✔ Abdominal swelling
✔ Localized tenderness
✔ Fatigue and weakness
Unlike simple appendicitis, pain may last several days and feel more diffuse.
Warning Signs 🚨
Immediate medical attention is required if there is:
🚨 Severe abdominal pain lasting more than 48 hours
🚨 High fever with chills
🚨 Increasing abdominal swelling
🚨 Persistent vomiting
🚨 Signs of sepsis (rapid pulse, low BP)
How Is Appendicular Abscess Diagnosed?
Accurate diagnosis requires imaging and laboratory evaluation.
✔ Blood tests (elevated WBC count)
✔ Ultrasound abdomen
✔ CT scan abdomen (most accurate)
✔ Physical examination
CT scan helps determine:
✔ Size of abscess
✔ Extent of infection
✔ Presence of perforation
Treatment of Appendicular Abscess
Treatment depends on abscess size and patient stability.
1️⃣ Conservative (Non-Surgical) Management
In selected stable patients:
✔ IV antibiotics
✔ IV fluids
✔ Close monitoring
✔ Pain management
Small abscesses may resolve with antibiotics alone.
2️⃣ Percutaneous Drainage
For larger abscesses:
✔ CT-guided drainage
✔ Placement of drainage catheter
✔ Pus removal
✔ Continued antibiotics
This approach helps control infection before definitive surgery.
3️⃣ Interval Appendectomy
After infection subsides (usually 6–8 weeks later):
✔ Elective laparoscopic appendectomy
✔ Prevents recurrence
✔ Minimally invasive approach
4️⃣ Emergency Surgery
Required if:
✔ Abscess ruptures
✔ Generalized peritonitis develops
✔ Patient deteriorates
✔ No response to conservative treatment
Surgical options include:
✔ Laparoscopic appendectomy
✔ Drainage of abscess
✔ Washout of abdominal cavity
✔ Open surgery in severe cases
Recovery & Post-Treatment Care
✔ Hospital stay varies (3–7 days typically)
✔ Gradual oral diet
✔ Completion of antibiotics
✔ Follow-up imaging if required
✔ Wound care
Most patients recover well with timely management.
Possible Complications If Untreated
❌ Generalized peritonitis
❌ Sepsis
❌ Intestinal obstruction
❌ Fistula formation
❌ Multi-organ failure
❌ Death
Early diagnosis dramatically reduces risk.
Prevention of Appendicular Abscess
✔ Early consultation for abdominal pain
✔ Avoid self-medication
✔ Timely appendicitis surgery
✔ Regular follow-up
Ignoring right lower abdominal pain is the most common cause of complications.
When Should You See a Specialist?
✔ Pain in right lower abdomen lasting >24 hours
✔ Fever with abdominal pain
✔ Vomiting with severe tenderness
