What Is Abdominal Tuberculosis?
Abdominal Tuberculosis (Abdominal TB) is a form of tuberculosis that affects the abdominal organs. It is caused by Mycobacterium tuberculosis, the same bacteria responsible for pulmonary TB.
In India, abdominal TB is relatively common due to the high prevalence of tuberculosis.
It can affect:
✔ Intestines (most common)
✔ Peritoneum (lining of abdominal cavity)
✔ Lymph nodes
✔ Liver
✔ Spleen
Abdominal TB can mimic many other diseases such as Crohn’s disease, cancer, or appendicitis — making early diagnosis challenging.
How Does Abdominal TB Spread?
✔ Spread from lungs via bloodstream
✔ Swallowed infected sputum
✔ Direct spread from nearby infected organs
✔ Reactivation of dormant TB
People with weakened immunity are at higher risk.
Risk Factors
✔ Previous TB infection
✔ HIV/AIDS
✔ Malnutrition

✔ Chronic kidney disease
✔ Immunosuppressive therapy

Many patients present late due to mild initial symptoms.

✔ Anemia
✔ Inflammatory markers
Imaging
✔ Ultrasound abdomen
✔ CT scan
✔ MRI
CT scan often shows:

✔ Enlarged lymph nodes
✔ Ascites
✔ Strictures
Ascitic Fluid Analysis (If Fluid Present)
✔ ADA levels
✔ TB PCR
✔ Cytology
Endoscopy & Biopsy
✔ Colonoscopy
✔ Tissue biopsy for confirmation
Definitive diagnosis often requires histopathological confirmation.
Treatment of Abdominal Tuberculosis
1️⃣ Anti-Tubercular Therapy (ATT)
Standard treatment includes:
✔ Isoniazid
✔ Rifampicin
✔ Pyrazinamide
✔ Ethambutol
Duration: 6–9 months (or as advised)
Most patients respond well to medical treatment.
When Is Surgery Required?
Surgery is needed if:
✔ Intestinal obstruction
✔ Perforation
✔ Abscess formation
✔ Severe strictures
✔ Diagnostic uncertainty
Surgical Procedures May Include:
✔ Resection of affected bowel
✔ Stricturoplasty
✔ Drainage of abscess
✔ Adhesiolysis
Minimally invasive techniques are preferred when feasible.
Complications If Untreated
❌ Bowel obstruction
❌ Bowel perforation
❌ Fistula formation
❌ Severe malnutrition
❌ Sepsis
Early medical therapy prevents surgical emergencies.
Recovery & Follow-Up
✔ Complete full course of ATT

Compliance with medication is critical for complete cure.
