MESENTRIC ISCHEMIA

Mesenteric Ischemia

Causes, Symptoms, Emergency Diagnosis & Advanced Surgical Treatment

Advanced Gastrointestinal & Emergency Surgical Care in Thane – By Dr. Ankur Bhanushali

What Is Mesenteric Ischemia?

Mesenteric ischemia is a life-threatening condition in which blood flow to the small intestine or large intestine is reduced or blocked. The mesenteric arteries supply oxygen-rich blood to the intestines. When this blood supply is compromised, the bowel tissue begins to suffer damage and may eventually die (bowel gangrene).

This condition requires urgent diagnosis and immediate treatment to prevent fatal complications.

Types of Mesenteric Ischemia

1️⃣ Acute Mesenteric Ischemia (Emergency Condition)

✔ Sudden blockage of blood flow
✔ Rapid progression
✔ High mortality if untreated

2️⃣ Chronic Mesenteric Ischemia

✔ Gradual narrowing of arteries
✔ Usually due to atherosclerosis
✔ Causes post-meal abdominal pain

3️⃣ Mesenteric Venous Thrombosis

✔ Blood clot in mesenteric veins
✔ Slower onset

4️⃣ Non-Occlusive Mesenteric Ischemia

✔ Occurs in critically ill patients
✔ Due to severe low blood pressure

Causes of Mesenteric Ischemia

1️⃣ Blood Clots (Most Common in Acute Cases)

✔ Heart rhythm disorders (e.g., atrial fibrillation)
✔ Recent heart attack
✔ Blood clotting disorders

2️⃣ Atherosclerosis

✔ Narrowed arteries due to cholesterol buildup

3️⃣ Severe Hypotension

✔ Shock
✔ Major surgery
✔ Severe dehydration

4️⃣ Hypercoagulable States

✔ Genetic clotting disorders
✔ Cancer
✔ Oral contraceptive use

Symptoms of Mesenteric Ischemia

Acute Mesenteric Ischemia

✔ Sudden severe abdominal pain
✔ Pain out of proportion to examination
✔ Nausea and vomiting
✔ Bloody stools
✔ Abdominal distension
✔ Fever (late stage)

Pain is often severe but abdomen may initially appear soft.

Chronic Mesenteric Ischemia

✔ Abdominal pain after meals
✔ Fear of eating
✔ Weight loss
✔ Diarrhea

Patients may describe “intestinal angina.”

Emergency Warning Signs 🚨

🚨 Sudden severe abdominal pain
🚨 Blood in stool
🚨 Persistent vomiting
🚨 Rapid heartbeat
🚨 Drop in blood pressure
🚨 Signs of shock

Immediate hospital care is critical.

How Is Mesenteric Ischemia Diagnosed?

Blood Tests

✔ Elevated lactate levels
✔ White blood cell count
✔ Metabolic acidosis

Imaging (Most Important)

✔ CT Angiography (Gold Standard)
✔ Doppler ultrasound
✔ Conventional angiography

Early imaging significantly improves survival rates.

Treatment of Mesenteric Ischemia

Treatment depends on severity and cause.

1️⃣ Emergency Stabilization

✔ IV fluids
✔ Oxygen
✔ Broad-spectrum antibiotics
✔ Blood pressure support

2️⃣ Blood Thinners

✔ Heparin therapy
✔ Prevents clot progression

3️⃣ Endovascular Treatment

✔ Catheter-directed thrombolysis
✔ Angioplasty
✔ Stent placement

Used in selected early cases.

4️⃣ Emergency Surgery

Required when:

✔ Bowel is non-viable
✔ Perforation suspected
✔ Severe obstruction

Surgical procedures may include:

✔ Removal of dead bowel segment
✔ Bypass surgery
✔ Temporary stoma (if required)

Delay in surgery can lead to sepsis and death.

Postoperative Care

✔ ICU monitoring
✔ Nutritional support
✔ Infection control
✔ Gradual diet advancement

Recovery depends on the extent of bowel damage.

Complications If Untreated

❌ Bowel gangrene
❌ Perforation
❌ Sepsis
❌ Organ failure
❌ Death

Mortality rate is high if diagnosis is delayed.

Prevention & Risk Reduction

✔ Control heart conditions
✔ Manage cholesterol
✔ Stop smoking
✔ Maintain healthy blood pressure
✔ Treat clotting disorders

Early risk factor management prevents recurrence.

When Should You Consult a Surgeon Immediately?

✔ Sudden unexplained abdominal pain
✔ History of heart disease with severe abdominal pain
✔ Persistent post-meal pain and weight loss
✔ Blood in stool

Do not ignore severe abdominal pain.
