MECKEL’S DIVERTICULUM

Meckel’s Diverticulum

Causes, Symptoms, Diagnosis, Complications & Advanced Surgical Treatment

Advanced Gastrointestinal & Laparoscopic Surgical Care in Thane – By Dr. Ankur Bhanushali

What Is Meckel’s Diverticulum?

Meckel’s Diverticulum is a congenital abnormal pouch present in the small intestine, usually located within 2 feet of the ileocecal junction. It is a remnant of the embryological vitelline duct that fails to disappear completely before birth.

It is the most common congenital abnormality of the gastrointestinal tract, affecting approximately 2% of the population.

Most individuals remain asymptomatic throughout life. However, in some cases, it can lead to serious complications requiring urgent surgical intervention.

The “Rule of 2s” (Classic Description)

✔ Occurs in 2% of the population
✔ Located about 2 feet from ileocecal valve
✔ Around 2 inches long
✔ Commonly presents before age 2 (but can occur in adults)
✔ May contain 2 types of ectopic tissue (gastric or pancreatic)

Why Does Meckel’s Diverticulum Cause Problems?

The diverticulum may contain ectopic gastric tissue, which can secrete acid. This acid can damage the nearby intestinal lining, leading to ulcers, bleeding, or perforation.

Complications usually arise due to:

✔ Inflammation
✔ Obstruction
✔ Bleeding
✔ Perforation

Symptoms of Meckel’s Diverticulum

Symptoms depend on the type of complication.

In Children

✔ Painless rectal bleeding
✔ Dark or maroon-colored stools
✔ Anemia
✔ Weakness

In Adults

✔ Abdominal pain (mimics appendicitis)
✔ Intestinal obstruction
✔ Nausea and vomiting
✔ Fever (if inflamed)

Some patients may remain asymptomatic and the condition is discovered incidentally during surgery or imaging.

Warning Signs 🚨

Immediate medical attention is needed if:

🚨 Fresh blood in stools
🚨 Severe abdominal pain
🚨 Persistent vomiting
🚨 Abdominal swelling
🚨 Signs of intestinal obstruction

Bleeding from Meckel’s diverticulum can sometimes be severe and life-threatening.

Complications of Meckel’s Diverticulum

1️⃣ Gastrointestinal Bleeding

Most common in children. Caused by acid secretion from ectopic gastric tissue.

2️⃣ Intestinal Obstruction

✔ Intussusception
✔ Volvulus
✔ Internal hernia

3️⃣ Diverticulitis

Inflammation that resembles appendicitis.

4️⃣ Perforation

Rupture leading to peritonitis.

How Is Meckel’s Diverticulum Diagnosed?

Diagnosis can be challenging and requires specialized tests.

✔ Blood tests (to detect anemia or infection)
✔ Ultrasound abdomen
✔ CT scan abdomen
✔ Meckel’s scan (Technetium-99m scan – especially in children)
✔ Diagnostic laparoscopy

The Meckel’s scan detects ectopic gastric mucosa.

Treatment of Meckel’s Diverticulum

Treatment depends on symptoms and complications.

1️⃣ Asymptomatic Cases

If discovered incidentally, surgical removal may be considered depending on:

✔ Age of patient
✔ Risk factors
✔ Diverticulum characteristics

2️⃣ Symptomatic Cases

Surgery is the definitive treatment.

Laparoscopic Diverticulectomy

✔ Minimally invasive
✔ Removal of diverticulum
✔ Faster recovery
✔ Minimal scarring

Segmental Bowel Resection

✔ Required if ulceration or perforation present
✔ Removal of affected intestinal segment

Emergency Surgery

Required in cases of:

✔ Severe bleeding
✔ Obstruction
✔ Perforation
✔ Peritonitis

Timely intervention prevents serious complications.

Recovery & Postoperative Care

✔ Hospital stay: 3–5 days typically
✔ Gradual oral intake
✔ Pain management
✔ Early mobilization
✔ Follow-up visits

Most patients recover completely after surgery.

Possible Complications If Untreated

❌ Severe blood loss
❌ Intestinal obstruction
❌ Perforation
❌ Sepsis
❌ Death (rare but possible in severe cases)

Early surgical treatment offers excellent prognosis.

When Should You Consult a Specialist?

✔ Unexplained rectal bleeding
✔ Recurrent abdominal pain
✔ Suspected appendicitis with normal appendix
✔ Intestinal obstruction symptoms

Early diagnosis prevents emergency situations.
